This contrast enhanced sagittal (from the side) T1 weighted MRI image of the brain shows extensive pathologic leptomeningeal enhancement in this person with sarcoidosis. Sarcoid is an inflammatory,multisystem,non-caseating granulomatous disease of unknown etiology. The prevalence is 10-20 cases per 100,000 people. African Americans have a 3-fold increased risk for sarcoid compared with whites. Most symptomatic patients are between the ages of 20 and 40. Neurosarcoid is an uncommon but severe manifestation of sarcoidosis. Neurosarcoid usually only occurs if the disease has had significant systemic involvement.
