Dermal Sinus and Tethered Cord (MRI)
This sagittal (from the side) STIR MRI image suppresses signal from fat making the fat look dark. This image shows the typical appearance of a dermal sinus tract which is a small,subtle tract extending from the skin of the back into the underlying tissues. It may terminate in the subcutaneous fat or extend all the way into the thecal sac of the spinal canal where it can cause multiple episodes of meningitis because of the connection with the skin surface. At the bottom of the spinal canal there is a cigar shaped black lesion. This black lesion is an intradural lipoma and it is tethering the spinal cord lower than normally within the spinal canal. The signal is black on this image because the specific MR sequence used suppresses fat signal on this sequence. The round bright signal focus on the skin surface of the lower back is a vitamin E capsule which is bright on MRI
